**COVID-19 FAMILY VACCINATION CLINIC** The St. Lawrence County Public Health Department is hosting a Family Vaccination Clinic — first dose Pfizer vaccine clinic for adolescents 12-17 years AND the parent/guardian accompanying the adolescent, on Saturday, May 22ND from 9am – 4pm at the St. Lawrence University Leithead Field House, by appointment only.
· If a child is not accompanied by a parent/legal guardian, the Parental Consent Form must be filled out.
