OptumRx (pharmacy) will cover the cost of at-home over the counter COVID-19 tests for employees on Clarkson’s health care plan effective January 15, 2022.
Reimbursement Highlights
- Tests bought on 1/15/22 and on are eligible for reimbursement
- All members on the plan are eligible for 8 tests every 30 days. Example: a family of four can claim 32 tests within 30 days
- The FDA approved tests for reimbursement are shown in the chart below
- Right now, only Walmart and Rite Aid pharmacies are processing at point of sale, all other pharmacies will require a member to pay out of pocket and submit a claim to Optum (claim form attached). Claims may also be submitted online here, https://covidtest.optumrx.com/covid-test-reimbursement, which is the quickest way to get reimbursed. For those unable to access via the OptumRx link can submit for reimbursement using the reimbursement form.
