In a communication to Associated Colleges colleagues, the St. Lawrence County Public Health Department has shared that COVID-19 vaccination eligibility can now include remote workers in higher education, who will be returning to work in person.
The St. Lawrence County Public Health Department will be administering COVID-19 vaccinations on Friday, March 19th from 8:00 AM – 6:30 PM at the Leithead Fieldhouse, 100 Park St, Canton, NY 13617. You must bring proof of employment— can be an employee ID, pay stub, letter from employer, etc. Contact HR at 315-268-6497 if questions. The University is giving all employees paid time off to make these appointments.
There are two steps to complete the registration process for a vaccine:
1. Registration: https://www.stlawco.org/EligibilityLeithead3.19
2. Attestation of Eligibility: Please complete the eligibility form prior to your vaccination: NYS Vaccine Form
Please bring proof that both the registration and the eligibility have been completed either by printing or taking a screenshot on your phone.
Please also review these guidelines to ensure you are eligible on the day of your vaccine:
• Wear a short sleeve shirt to expose your upper arm for the vaccination
• If you are currently in isolation or quarantine due to COVID-19, you are not eligible. You will need to schedule at a later date upon availability.
• You must be at least 18 to receive the Moderna vaccine.
• You must be available for the second dose on April 16th.
• If there are no slots left when you go to register, please let the Public Health Department know by sending an email with your name, phone number, and the location you are trying to sign up for to SLCCOVID19SharedMailbox@stlawco.org and you will be placed on standby.
Once you have received both vaccines (Pfizer or Moderna) or your single vaccine (Johnson & Johnson), please provide Clarkson with your vaccine documentation (https://intranet.clarkson.edu/peoplesoft/ – HR, click vaccine proof, upload photo of vaccine record). We believe it is important to have employees provide this information to Clarkson because, based on conversations with our local medical experts regarding in-person activities, this information is useful in these deliberations. Your vaccination information will not be shared with anyone except HR.
