Healthcare is essential for all of us and our families. It is important for promoting and maintaining
health, preventing and managing disease, reducing unnecessary disabilities and so much more. The cost
of healthcare continues to rise impacting both employer and employees’ premium costs as well as
overall out of pocket expenses.

The data collected from this questionnaire will help guide the University in evaluating our current
healthcare coverage against other available options and what the cost impact will be for employees and
the University.
Please note that this is routine and intended to help us assess your needs and concerns. We are not
contemplating any immediate changes to our health care plan, and further discussion and deliberation
would certainly take place if any changes were seriously considered.
